Create a one-of-a-kind smash book to preserve memories or use as a journal! All book supplies provided, but please bring any photos and/or small mementos you might want to include.
Participants may be recorded and/or surveyed to capture the program's effectiveness. If you do not want your teen recorded and/or surveyed, please fill out the Audio/Visual Recordings Opt-Out form and/or the Surveys Opt-Out form. This program may include activities that could be considered harmful. In order for your teen to participate, a Release and Hold Harmless form must be signed by parent or legal guardian and returned to the program location at least one week prior to program start date.
