The Release & Hold Harmless Form must be completed and returned one week prior to the start of the program. The completed form must be returned to the program location.
You may drop the completed form off in person, send it by fax or email a copy to the attention of the Branch Manager
at the program location.
The Release & Hold Harmless Form can be found here.
