This program may include activities which could be considered harmful. In order for your tween to participate, a parent/legal guardian must sign the Release and Hold Harmless form and return to the program location at least one week prior to the program start date.
Participants may be photographed/recorded and/or surveyed to capture the program's effectiveness. If you do not want your tween to be photographed/recorded and/or surveyed, please fill out the Audio/Visual Recordings Opt-Out form and/or the Surveys Opt-Out form.
